7th Grade Health Packet 

• Physicals are mandatory for all incoming 7th grade students. All required forms can be located in the District Registrar section.

• Asthma, allergy and medication forms are to be returned to the MS nurses office no later than the first day of school (if applicable).

• If your student is participating in Intramurals, please submit the physical on the first day of school to the nurse.

• A physical examination must be submitted to participate in any of the Interscholastic Sports listed below. Send the exam to RDHS Athletic Department.  All physicals will be returned to the MS nurse's office after being cleared by the school doctor.
