8th Grade Health Packet

For new incoming 8th grade students, please visit the District Registrar page for all required forms.
Asthma, allergy and medication forms are to be returned to the nurses office on the first day of school (if applicable).
If your student is participating in Intramurals, please submit the physical on the first day of school to the MS nurse.
A physical examination must be submitted to participate in any of the Interscholastic Sports listed below. Send the exam to RDHS Athletic Department. All physicals will be returned to the MS nurse's office after being cleared by the school doctor.
